The Lead Care Manager serves as the leader of a group of Care Managers/Designated Care Managers during the shift. The Lead Care Manager ensures that quality care and services are provided and documented, consistently and in alignment with each resident’s individualized Service Plan (ISP) and Sunrise policies, for residents in our care neighborhoods.
The Service Manager leads by example in all job responsibilities and may work variable hours, possibly including some weekend and holiday duties.
Essential Responsibilities:
- Provide supervisory support by taking attendance at the start of each shift; managing call-outs/no-shows; sending open shift messages and responding to shift requests
- Support team members in providing Activities of Daily Living (ADL) care and services to residents in accordance with their Individualized Service Plans (ISP).
This may include bathing, personal and dental hygiene, bowel and bladder functions, dressing, hair care, escorting to and from meals and activities, and tidying the resident’s room.
- Support team members as they encourage personalized and meaningful resident participation in life enrichment programs and ADLs
- Assist with food service in the dining room. Instruct team members on hospitality standards during meal service.
Accurately document care and services provided to residents. Review care manager documentation throughout the shift. Identify and fill gaps in documentation by directing staff members - Ensure compliance with Sunrise’s Time Tracking and Meal/Rest Period policies. Monitor team members’ break schedule and duration;
Ensure that service managers clock in/out using the correct job codes. Train team members as needed
- Demonstrate and encourage flexibility in working hours, working holidays, evenings, weekends and additional shifts when necessary
- Prepare for the shift by obtaining resident group assignments before care managers arrive. Print and distribute daily assignment sheets to appropriate staff members.
- Participate in cross-functional meetings sharing/receiving pertinent information with/from other Key Account Managers
- Perform resident group duty on night shifts and take over group duty in case of unfilled call on day and night shifts
Other responsibilities
- Trained and credentialed (including certifications when required by the state) to safely administer medications if needed during periods of high volume or unexpected absence
- Set an example by clocking in/out and taking appropriate meal/rest breaks
- Participate in the group interview process, including a job preview video and community tour
- Support the onboarding of new team members, including coaching and skills demonstration
- Maintain a safe environment for all employees, residents and guests by following established safety standards; actively supporting safety practices
- Report incidents and complete appropriate paperwork immediately.
- Communicate any observed or suspected changes in resident condition to the departmental care coordinator; assist care coordinators, as needed, in developing and/or updating ISPs and communicating with residents and families
- Prevent and manage conflicts by working with staff toward a resolution. Communicate issues to the department care coordinator when necessary
- Contribute to the Care Manager’s annual performance review by providing feedback to the department’s Care Coordinator
Project a positive, professional and friendly image through actions, words and dress
Qualifications:
- High school diploma or GED strongly preferred and may be required by state regulations
Ability to read, write and communicate effectively with care managers, community managers, residents and family members - In states where appropriate, you must maintain applicable certifications
- Must be at least 18 years old
- Medication Management Certificate required; ongoing training and re-education per Sunrise and state requirements
- Previous experience working with seniors and a desire to serve and care for seniors
Demonstrates leadership skills - Ability to delegate tasks to appropriate individuals based on their skills, roles and interests
- Ability to make choices, decisions and act in the best interests of residents
- Ability to react appropriately and remain calm in difficult situations
- Ability to deal with multiple priorities
- Proficient in organizational and time management skills
- Demonstrate good judgment, problem-solving, and decision-making skills
- Computer literate, Microsoft Office and Sunrise applications with the ability to learn new applications
